Information & Referrals
The Columbia Heights/Shaw Family Support Collaborative (CH/SFSC) provides information and referrals for services to families.
We connect with these families through the Child and Family Services Agency, through community-based organizations, and through the families themselves.
CH/SFSC works with a variety of service providers to provide a network of support for residents. We facilitate access to such services as:- housing and utilities assistance
- employment assistance
- mental health services
- emergency provision of food and clothing
- Family group decision making
- We assisted 546 individuals.
- 88% were successfully matched to a provider or referral source.
